As you may be aware, significant changes in the VA Suicide Risk Identification Strategy (Risk ID) were communicated a little over a year ago to inform the field about new processes and requirements for VA suicide risk identification (see attached). This memo states that all Veterans should be screened annually for suicide risk. The annual suicide risk screen reminder should be satisfied by appropriate staff, at a Veteran’s encounter, when it is due. The Suicide Risk Identification and Management SharePoint site has many useful resources, including, staff specific guidance, training, lethal means safety, and Suicide Prevention Clinical Tips. CPRS contains a template of the Columbia-Suicide Severity Rating Scale (C-SSRS) Screener which is prompted via the clinical reminder system.
Those with a positive C-SSRS screen require same-day completion of a Comprehensive Suicide Risk Evaluation, which is often completed after a warm hand-off to a mental health professional.
VA has developed a tool to help monitor completion of the C-SSRS, and track missed opportunities for completion. Many eye care providers are clustered under one heading “Ophthalmology & Optometry” in the analysis tool. This heading collectively includes stop codes:
- 407 –OPHTHALMOLOGY
- 408 –OPTOMETRY
- 428 –TELEPHONE/OPTOMETRY
- 437 –VICTORS & ADVANCED LOW VISION
- 438 –INTERMED LOW VISION CARE
- 439 –LOW VISION CARE
- 718 –EYE TELE SCREENING.
